Overview:

A healthy community starts with access to nutritious food and reliable health benefits. Our Food & Health Services program helps Queens residents navigate essential support systems like SNAP, Medicaid, HEAP, and Fair Fares. We offer one-on-one case management to connect families with food programs, healthcare coverage, and energy assistance while also hosting seasonal food distributions to meet immediate needs.

Our goal is to ensure that no neighbor has to choose between paying rent and putting food on the table. We walk clients through the often confusing application process and advocate for resources that strengthen community health and stability.

Do I need an appointment?

Yes, we are appointment only and our sessions are scheduled for 60 minutes.

Who is eligible for your services?

Our services are free and available to all Queens residents, regardless of immigration status. If you reside outside of Queens, we will help connect you with the appropriate agency.

What documents should I bring?

We recommend bringing a photo ID, proof of address, and any documents related to the issue you need help with. If you're unsure, we’ll guide you on what’s needed.

Do you offer services in other languages?

Yes, we provide support in multiple languages and can connect you with interpreters when needed.

What other issues can your case managers help with?

Outside of food and health services, our team supports tenants’ rights issues, access to public benefits, community resources, and legal consultation for immigration / housing concerns.
