Please be informed that enrollment for our After-School Program for September 2011-June 2012 ended on Friday, May 27, 2011. We are no longer accepting enrollment requests.

For parents who submitted an application to be part of the lottery, the drawing will be held on:

Wednesday, June 15, 2011 at 6:30pm for PS 152

and

Tuesday, June 21, 2011 at 6:30pm  for PS 11

Please try to attend the lottery if you have submitted a lottery application. Those who are selected during the lottery will be provided a program application at that time. For those who are not present at the lottery but selected for the program, they will be notified by phone and/or by mail to pick up a program application, but must wait to be contacted.

Completed applications (including medical) are due back to the WOTM main office by Friday, July 1, 2011.  **Applications will not be accepted after that time.**

FOR CHILDREN CURRENTLY 5-12 YEARS OLD IN GRADES K-6

Please complete the lottery form if you wish to enter your child/ren into the lottery for the 2011-2012 Woodside on the Move, Inc., P.S. 11 Q or P.S 152 Q Fall After-school Program.

One lottery application per family. Please list all the children in the family on the form. A copy of a birth certificate for each child on the application will be required.

PLEASE DO NOT SUBMIT MORE THAN ONE LOTTERY APPLICATION PER FAMILY.

Lottery applications will be available for pick up and drop off at the

Woodside on the Move, Inc. main office, located at 39-42 59^th Street, 2^nd Floor, Woodside, NY

beginning Monday, May 16^th through Friday, May 27^th,

between the hours of 9:00am to 5:00pm.

Lottery forms will also be available for pick up and drop off until 6:00pm on

Thursday May 19^th, 2011 and Thursday May 26^th, 2011.

All lottery applications, as well as verification of child’s age (birth certificate) must be returned to the Woodside on the Move, Inc. main office

by Friday, May 27^th, 2011, 5:00pm.

Applications will not be received at the school.

LOTTERY APPLICATION DEADLINE MAY 27^th, 5:00PM (NO EXCEPTIONS)

The P.S.11 Q Lottery Drawing will be conducted at the school site located at

54-25 Skillman Avenue, Woodside, NY 11377 on

Tuesday, June 21, 2011, 6:30pm-8:00pm.

The P.S. 152 Q Lottery Drawing will be conducted at the school site located at

33-52 62^nd Street, Woodside, NY 11377 on

Wednesday, June 15, 2011, 6:30pm-8:00pm.

75 additional lottery applications will be called and placed on the waiting list for each school.
