Thank you for your interest in our after-school program for the period September 2010- June 2011. If you have submitted your child’s/children’s name(s) for the after-school program lottery, you are invited to attend the name drawing and registration.

TIME: 6:00pm
If selected please plan to stay as you will be asked to complete the enrollment application at that time.
You do not need to be present to be selected. We will also be scheduling appointments to complete the applications for parents who are unable to attend.

Dear Parents:
Due to the overwhelming response regarding our PS 11 OST afterschool program we have no choice and have decided to change the registration process to a lottery. Please complete the attached form below in its entirety. Please make sure your contact information is legible and clear as we will have to contact you.
These contact forms will be accepted until 7:00pm on Friday, April 30th , 2010. Three-hundred (300) names will be drawn. Only one form will be accepted per family and you may only request to register your own children. Parents will then be notified during the next two weeks to schedule an appointment to complete a registration application.
We apologize for any inconvenience this may cause, however registration has become an ongoing challenge due to early lineup.

Estimados padres:
Debido a las muestras de interés de los padres en nuestro programa después de la escuela PS 11 OST, hemos decidido cambiar el proceso de registración a una lotería. Por favor llene este formulario completamente. Asegúrese que la información que escribe se lee con claridad para poder contactarles.
Los formularios con la información serán aceptadas hasta el día viernes 30 de abril a las 7:00PM. Se seleccionaran 300 nombres de la lotería. Los padres serán notificados en un periodo de dos semanas para que saquen citas y puedan llenar un formulario de solicitud.
Nuestras disculpas por cualquier inconveniente que pudiéramos causar. Aún si, el proceso de registración ha sido un reto permanente debido a la respuesta temprana a nuestra convocatoria.

In response to the devastating events in Haiti, Woodside On The Move, Inc., donated 100% of the proceeds raised in their fundraising campaign, Health For Haiti. Funds were generated though the sales of healthy snacks. The children and the families in our DYCD funded OST after-school programs at PS 11 and PS 152 showed a tremendous response and commitment to ensuring that our goal of assisting the people of Haiti to help them in any way to recover from the massive Earthquake that struck the Island in January 2010 would be accomplished.
Woodside On The Move, Inc., as well as the children and families that attend our after-school programs, hope that this donation will be utilized to aide in the recovery effort and to help provide the necessary resources for the children and families of Haiti.

Completed medical forms must be returned to the Woodside On The Move Main office during the week of May 17 – 21, 2010, between the hours of 10:00am – 1:00 pm. All children must have a recent physical examination form signed by a Doctor.

FREE SUMMER CAMP PROGRAM
For children currently 5 – 12 years old (DOB must be prior to June 30, 2005 for Kindergarten and for grade 6 DOB must be after August 31, 1997)
Program activities include:
sports, crafts, arts, games, trips, computers, community beautification,
movies, music, and other cultural activities.
Breakfast and lunch provided. Parents are required to provide a nutritious daily snack.
Summer Camp Program:
July 2010 – August 2010
Mondays to Fridays – 8:00 AM to 5:30 PM

Program Registration will take place on a first-come, first-serve basis
Summer Camp Registration will be held:
Saturday, March 27, 2010 from 10:00 AM to 3:00 PM

Parent/Guardian must provide verification of child’s age and ID for affiliation purposes. Parent/Guardian will only be able to represent one family/household.
Therefore one representative is needed for each family/household.
Children accepted into the Summer Camp Program will also be eligible to participate in the Fresh Air Fund sleep away camp or Friendly Town Programs.
